★ ★ ★ ★ ★ Extremely expensive! Charged $70 in total to sign an assistance form. $45 signing and $25 consultation despite of my mother being on financial assistance. In the end the form was rejected. Paid a heavy price to learn a lesson to not visit this overpriced clinic again.
